Contact Luminary Home Care
Call us or fill out our contact form. We'll listen carefully to your situation, answer your questions, and explain how Home Help can support you or your loved one — with no pressure and no obligation.
Verify Medicaid Eligibility
Home Help is funded through Michigan Medicaid. We'll help confirm that the client is enrolled in Medicaid and point you in the right direction if additional steps are needed to establish coverage.
Notify Your MDHHS Adult Services Worker
Contact your local MDHHS office to request Home Help services. An Adult Services Worker (ASW) will be assigned to begin the assessment process and determine eligibility for care.
Authorization Meeting
The ASW visits the home to assess needs and authorize a set number of care hours. Luminary can attend this meeting with you, providing our CHAMPS Provider ID so we can be designated as your chosen provider.
Meet Your Caregiver
We carefully match you with a compassionate caregiver suited to your needs, personality, and routine. You'll have the chance to get acquainted and feel comfortable before care officially begins.
Services Begin
Your caregiver begins providing the authorized services on your schedule. We stay in close contact, monitor quality, and adjust care as needs change — so support always fits your life.
Referring a Client to Luminary
A streamlined process for discharge planners, case managers, and MDHHS caseworkers.
Send Us the Referral
Reach out by phone or email with the client's basic information and care needs. We respond quickly and can begin coordinating services right away — even ahead of a hospital discharge.
Provide Our CHAMPS Provider ID
Share our CHAMPS Provider ID — 6405288 — so the client can name Luminary Home Care as their chosen Home Help provider. This ensures a smooth designation when authorization is processed through Michigan Medicaid.
ASW Authorization Meeting
The client's MDHHS Adult Services Worker conducts the needs assessment and authorizes care hours. We coordinate scheduling and attend as needed to ensure a seamless handoff from your care to ours.
Care Begins & Updates Flow
Once authorized, we deploy a matched caregiver promptly and document visits through EVV. We're happy to keep referring professionals informed so you have confidence the client is well supported.
